Onboarding Comprehensive Medication Consultation

$275 - One-Time

A comprehensive 90-120 minute assessment: medical history, medication reconciliation, specialist review, and a baseline care plan that you can understand.

Additional Visits

$75 - Per Visit

For additional in-home or in-office visits - the same preparation, clinical advocacy and written follow-through, as often as you need it.

Annual Plan - First Year

$4500

Onboarding Medication Therapy Consultation plus twelve months of subscription support paid up front- $3900 per year thereafter.

Monthly Subscription

$275 - Per Month

Ongoing navigation and medication management-an always current medication profile, coordination among your providers, and regular communication.

Point of Care Testing and Prescribing

These test can be performed in your home and prescription sent to the pharmacy and we will pick the prescription up and deliver to you. (Additional $50 for in home and medication delivery)

Medical assessment $60

Includes a symptom review, thorough health screening and evaluation by a certified POC testing pharmacist to determine your eligibility for testing and treatment.

*Charged for all visits regardless of whether a test is performed or medication is needed.

Strep Test $40

A test for strep that involves swabbing the back of the throat.

Total for Strep Test and Prescribing Treatment if necessary: $100

Flu A/B+COVID+ RSV Test $40

The combo test involves swabbing the inside of the nose.

Total for FluA/B+COVID+RSV Test and Prescribing Treatment if necessary: $100

All other appointments (altitude sickness, GLP-1 medication management, Accutane side effect management and education, prescribing for other Colorado approved conditions, etc.) $75

All appointments may be performed at your home or in our Mountain Top Rx office in town on Main Street.
